Show me the registration forms!

Hey people! I need your help. Last Sunday I picked up and looked at our “registration card” that first time visitors fill out. Embarrassingly, it was the first time I looked at this. I’ve got several new volunteers who are doing our data entry and I was overseeing their training. I tried to enter one of these cards into the database and found myself guessing and making assumptions. Unfortunately, our registration card causes you to guess who the cell phone belongs to and who the email belongs to. There are other things as well, but I won’t get into them.

So, I plan to revamp this sucker in the next few weeks. Here’s where I could really use your help. I love to look at how other churches do their cards. What information do you ask for? How big is the card? What is they layout like?

If you would, send me your card. Take a picture of it, scan it or send me the original file. When I get it, I’ll create a new post and upload all your examples. Here’s a great opportunity to share and learn from each other… and it sure would help me out a lot!

    I created mine on a notepad you tear off the sheet and there is one on the bottom. I ask for basic info for F1 to keep the line moving. I also have a special line maned by a Kid Concierge. Their job is to help assist and check in new families.

    Oh and IMHO that from is to long. Cut it down. Just what you need to keep the kiddo safe and fulfill what you need for your database.

    Our insurance company ask for much, much, much more. I’m required to have way too much information! I need to clarify that again, but last year I was unable to change it that much.

    I just our MHK registration form to Kenny. Our challenge is that we have so many kids who are not brought by their parents so when we enter them into our database they are connected to the wrong adults, wrong address or none at all. We currently use Shelby for attendance tracking and rosters, but will be moving over to an Arena check-in that famiies will do themselves. Capturing accurate information from the beginning without it being to cumbersome is critical.
