New family registration form: Redux

Okay, you’re going to start thinking that I like to re-design registration cards for fun. Actually, it is kind of fun. So back in October, we redesigned the registration card we were using. Functionally speaking, it was just about perfect. However, as soon as it was done and after I posted it here, I realized something I was missing. The Children’s Ministry at Gateway has had somewhat of an “old world” feel thing going on. It looks great, but we’re wanting to move the look and feel in a different direction. Something more vibrant, fun and colorful. So I don’t know what I was thinking by redesigning our registration card to look “old world.” So, I sent this back to our graphic artist and she changed the feel. It’s a two sided card (doesn’t cost hardly anything to print on the other side) so we had our logo (the one we’re using now until re totally re-brand) splashed on the back. Let me know what you think.

BTW, on the last post, someone asked about what we do if a friend brings someone and how we handle that. Next week I’m working on a supplement to handle that as well as keep our records accurate.

New family registration form

Back in February I wrote about family registration cards. Our card was old, out-dated and way to vague. We had the card redesigned and have been using it for several months now. Actually, the new card has even undergone a few revisions. With forms like these, I tend to print them in house and use them until we discover all the mistakes or things that need to be adjusted. You won’t really know until you use them. Once we’re happy, we have them professionally printed.

So, here’s the card we ended up with.

What do you think? Any changes that you would suggest?

Hey people! I need your help. Last Sunday I picked up and looked at our “registration card” that first time visitors fill out. Embarrassingly, it was the first time I looked at this. I’ve got several new volunteers who are doing our data entry and I was overseeing their training. I tried to enter one of these cards into the database and found myself guessing and making assumptions. Unfortunately, our registration card causes you to guess who the cell phone belongs to and who the email belongs to. There are other things as well, but I won’t get into them.

So, I plan to revamp this sucker in the next few weeks. Here’s where I could really use your help. I love to look at how other churches do their cards. What information do you ask for? How big is the card? What is they layout like?

If you would, send me your card. Take a picture of it, scan it or send me the original file. When I get it, I’ll create a new post and upload all your examples. Here’s a great opportunity to share and learn from each other… and it sure would help me out a lot!

UPDATE: I’m posting the cards you send me here.

This is the card Gateway has been using for at least the last several years. It’s 5.5 x 4.25.
